Our Comprehensive Home Health Care Services

At Health Force of Georgia, we offer a full spectrum of in-home services designed to meet diverse needs—from medical support to daily companionship. Delivered by skilled professionals, our care ranges from a few hours weekly to round-the-clock assistance. Here's an overview:

  • Home Infusion Therapy - Safe, convenient IV treatments for antibiotics, nutrition, pain management, and more—administered by trained nurses in the familiarity of home.
  • Companion Care - Warm, engaging companionship to combat loneliness, including conversation, light activities, outings, and emotional support.
  • Personal Care Assistance - Gentle help with daily activities like bathing, grooming, dressing, toileting, and mobility—always with respect for privacy and dignity.
  • Skilled Nursing Care - Advanced medical oversight by RNs and LPNs, including wound care, IV therapy, medication management, chronic condition monitoring, and post-surgical support.
  • Respite Care - Temporary relief for family caregivers, providing short-term in-home or facility-based care so you can recharge without worry.
  • Alzheimer’s & Dementia Care - Specialized, patient-centered support for memory challenges, including safety monitoring, engaging activities, and strategies to maintain routine and comfort.
  • GAPP Care for Children - Pediatric services through Georgia's Medically Fragile Children Program, offering skilled nursing and personal care for kids with complex medical needs.

Spotlight: Structured Family Caregiving (SFC) – Empowering Families to Care at Home

One of our most impactful programs, Structured Family Caregiving (SFC) is a Medicaid-funded option under Georgia's Elderly and Disabled Waiver Program (EDWP). Available through the Community Care Services Program (CCSP) or Service Options Using Resources in a Community Environment (SOURCE) waivers, SFC allows eligible family members to become paid caregivers for loved ones needing nursing home-level care—while keeping them at home.

What is SFC? SFC turns dedicated family caregiving into a supported, compensated role. Eligible relatives (by blood or marriage, excluding spouses or legal guardians) provide daily assistance with activities like bathing, eating, medication management, and mobility. In return, caregivers receive a tax-free weekly stipend (e.g., around $463 per family member or $926 for two), direct-deposited as Health Force of Georgia employees.

Benefits of SFC:

  • Financial Support: Tax-free payments ease the economic strain of full-time caregiving.
  • Professional Guidance: Access to a dedicated health coach or nurse for personalized care plans, monthly check-ins, and emotional support.
  • Training and Resources: At least eight hours of annual customized training, plus tools like emergency response systems and home modifications.
  • Family-Centered Healing: Loved ones receive care from trusted relatives, promoting emotional security, independence, and community ties.
  • No Out-of-Pocket Costs for Eligible Families: Fully covered by Medicaid waivers, with potential income-based cost-sharing.
  • Peace of Mind: Regular oversight, daily note reviews, and 24/7 access to our clinical team with over 30 years of experience.
