Health Force of Georgia is a Georgia-approved, participating provider of at-home care services to adults eligible to receive the CCSP waiver. This means persons who qualify for the CCSP program may receive homecare or skilled nursing care services in their homes from Health Force of Georgia at no charge, or a small monthly cost-share. Health Force of Georgia is reimbursed for services directly by the State of Georgia.

- Residency: Live in Georgia.
- Age or Disability: Be 65 or older, or under 65 with a qualifying physical disability (those enrolled before 65 can continue after turning 65).
