What is the Structured Family Caregiving (SFC) Option?
Structured Family Caregiving is a service option under Georgia's Elderly and Disabled Waiver program. It is designed to support family caregivers who live with and provide daily care for a senior or a person with disabilities. By offering financial compensation and professional health coaching, the program helps caregivers focus on delivering personalized, loving care in the safety of their own home.
The SFC Option is available through Georgia’s Community Care Services Program (CCSP) or Service Options Using Resources in a Community Environment (SOURCE) waivers, ensuring eligible Medicaid recipients can remain in their communities with the support of those who know them best. The SFC Option is ideal for families who want to avoid institutional care while ensuring their loved one receives high-quality, compassionate support. Caregivers receive a tax-free weekly stipend, ongoing training, and access to a dedicated health coach to guide them through their caregiving journey.
As a Caregiver under our SFC Program,
You’ll Receive:
- Tax-free, financial stipend paid weekly and direct deposited for your caregiving services
- Coaching and guidance from our professional health coach
- Daily personalized care plan to follow developed for your loved one by our medical staff
- Telephone and email access to direct connect you with your health coach
Who Qualifies for SFC ?
To participate in the SFC Option, both the caregiver and the care recipient must meet specific eligibility criteria:
Care Recipient Requirements:
- Medicaid Eligibility: Must be enrolled in or eligible for Georgia Medicaid and participate in the CCSP or SOURCE waiver program.
- Care Needs: Requires assistance with daily living activities such as bathing, dressing, eating, mobility, or medication management, typically at a level that would otherwise qualify for nursing home care.
- Living Arrangement: Must live in the same household as the primary caregiver.
- Age or Condition: Must be 21 years of age or older and have a diagnosis causing physical limitations that meet Nursing Home level of care as defined by Georga Medicaid.
- Cost Share: May need to pay a monthly cost share based on income, if applicable.
Caregiver Requirements:
- Relationship: Must be related to the care recipient by blood (e.g., daughter, grandson) or marriage (e.g., daughter-in-law, son-in-law). Spouses and legal guardians are not eligible.
- Residency: Must live in the same household as the care recipient.
- Age: Must be 18 years or older.
- Employment Status: Cannot be employed outside the home or run a business from home, as caregiving must be the primary focus.
- Background Check: Must pass a criminal background check.

How to Apply for the SFC Program
Applying for the SFC Program is a straightforward process, and our team is here to guide you every step of the way. Follow these steps to get started:
Determine Eligibility
Contact us using the form on this page or call us at (770) 458-8500 ext. 812 or contact us by email to speak with an SFC advisor who will help assess your eligibility. Health Force of Georgia's SFC specialist will help you to determine if you qualify for the program.
Submit an Application
Provide necessary information, including medical records (e.g., a recent History and Physical from the care recipient’s primary care provider or specialist), to initiate the referral process.
Case Management Company Assigned
A Georgia Case Management Company will be assigned based on your location in Georgia.
Complete a Comprehensive Assessment:
A state-approved assessor will evaluate the care recipient’s physical, emotional, and cognitive needs to determine the level of care required.
Develop an Individual Service Plan
Work with a case manager, caregiver, and care recipient to create a personalized plan outlining services and support.
Select a Provider
Choose an approved SFC provider (like us!) to serve as your health coach company, facilitating stipend payments and ongoing support. Once approved, you'll become an employee of Health Force of Georgia and receive weekly payments.
HFGA as Your Coach
By choosing Health Force of Georgia as your health coach, you will get paid by HFGA on a weekly basis to care for your loved one. We give you ongoing support with a care coordinator or nurse, including monthly Health Coach Calls.
Professional Guidance
Get training, coaching, and support from HFGA's experienced health professionals with over 30 years of experience, so that caregivers are equipped to handle daily tasks and evolving care requirements.
Personalized Care
With the assistance of Health Force of Georgia's SFC specialists, you will follow a daily care plan developed specifically for your loved one by our medical staff. You are never alone. You have someone to call when you need help!

Georgia’s Structured Family Caregiving program is designed to help families stay connected and supported. Once you are approved for the Structured Family Care Program, you will officially become a participant in Health Force of Georgia as the primary caregiver. You'll receive weekly payments for your dedicated service, directly deposited into your account.
CCSP helps frail elderly or disabled individuals stay at home with coordinated care, preventing unnecessary moves to facilities. It's a nursing home diversion program that values your loved one's independence.
SOURCE links primary medical care with community services for frail elderly and disabled adults, promoting better health outcomes through integrated support.
Get paid for taking care of your loved ones. SFC allows relatives to become paid caregivers while receiving training and resources. It's available under waivers like CCSP and SOURCE, offering financial relief and emotional support for families caring for seniors or disabled loved ones at home.
