Who Qualifies for SOURCE?
To enroll in SOURCE, you must meet these eligibility criteria:
- Residency: Live in Georgia.
- Age or Disability: Be 65 or older with a chronic medical condition, or under 65 with a qualifying physical disability (eligibility continues after age 65).
- Medicaid Eligibility: Qualify for Medicaid and Supplemental Security Income (SSI). In 2025, the income limit for a single applicant is $943/month (SSI/Federal Benefit Rate) and $1,415/month for couples, with an asset limit of $2,000 for individuals ($4,000 for couples). A Community Spouse Resource Allowance of up to $154,140 applies for non-applicant spouses.
- Functional Need: Require a nursing home level of care due to physical or cognitive impairments, as confirmed by a physician through an assessment of daily living activities (e.g., bathing, dressing, mobility).
- Community Preference: Choose to receive care at home or in a community setting rather than a nursing facility.
SOURCE is not an entitlement program, with around 49,398 slots under the Elderly and Disabled Waiver (which includes SOURCE and CCSP). A waitlist may apply, prioritized by urgency of need, so apply early to secure your spot.
