Why Choose GAPP?
Empowering Families to Thrive at Home

Caring for a child with complex medical needs can feel overwhelming, but GAPP offers a beacon of hope by prioritizing home-based care over institutional settings. This program, managed by the Georgia Department of Community Health (DCH), supports medically fragile children with multiple system diagnoses, enabling them to stay in their communities where they can flourish emotionally and physically. Studies and family experiences show that in-home care promotes better health outcomes, reduces hospital stays, and strengthens family bonds—allowing your child to play, learn, and grow in a loving environment.

Unlike facility-based options, GAPP is flexible and family-centered, often at no out-of-pocket cost for Medicaid-eligible families. You're doing an incredible job, and GAPP is designed to support you, making every day a little brighter.

Core Services: Tailored Care with Heart and Expertise

GAPP focuses on skilled nursing and personal support to meet your child's unique needs, always delivered with compassion. Health Force of Georgia provides a range of services, blending professional care with the warmth of home. Here's an overview, inspired by official guidelines and best practices from top providers:

Skilled Nursing Services:

Provided by Registered Nurses (RNs) and Licensed Practical Nurses (LPNs), including monitoring vital signs (blood pressure, pulse, respiration, temperature), IV therapies, wound care, tube feedings, care for gastrointestinal disorders, tracheostomy and ventilator management, medication administration, and support for neurological or endocrine conditions.

Behavioral and Developmental Support:

Tailored help for conditions such as cerebral palsy, Spina bifida, paraplegia, autism, or intellectual/developmental disabilities (IDD), including behavioral interventions and companionship.

Personal Care Support:

Assistance with activities of daily living (ADLs) like bathing, grooming, dressing, feeding, mobility, transfers, housekeeping, and incontinence care, often handled by Certified Nursing Assistants (CNAs) or Home Health Aides (HHAs).

Additional Supports:

Care coordination, respite for caregivers, emergency response systems, home modifications, specialized equipment, and even school nursing where a nurse accompanies your child to class for seamless education.

Services are approved based on medical necessity, typically ranging from 25-40 hours per week, with 24/7 availability for peace of mind. We emphasize personalized plans that evolve with your child's needs, ensuring dignity and joy in every interaction.

Eligibility: Who Can Benefit from GAPP?

GAPP is open to children who meet specific criteria, focusing on those who would otherwise require hospital or skilled nursing facility care. Here's a clear breakdown, based on official Georgia Medicaid guidelines:

Age Under 21 years old (up to 20 years and 11 months in some references).
Medical Condition Medically fragile with multiple system diagnoses, requiring skilled nursing level of care (e.g., respirator/oxygen dependent, tracheostomies, IV therapies, neurological disorders, cardiac defects, or IDD).
Financial Must be eligible for Georgia Medicaid; family income is considered, but waivers like Katie Beckett* can help if the child qualifies independently.
Functional Needs Physician-documented need for institutional-level care, but able to live safely at home with support; no primary mental health diagnosis as the focus.

If your child has conditions like cerebral palsy, Spina bifida, or complex chronic illnesses, GAPP could be a perfect fit. Eligibility assessments are straightforward, and we're here to help you determine if your family qualifies.

* In Georgia, the Katie Beckett program (often called TEFRA/Katie Beckett) is available for children under 19 with disabilities who need substantial care but can remain at home. Eligibility focuses on the child's medical needs and functional limitations, not just family income—though assets are still considered, and the child must qualify for Medicaid in other ways. It can complement programs like GAPP by providing broader Medicaid coverage for therapies, equipment, and other services.
